Green Prescription supports positive health and lifestyle changes
Green Prescription is a free service which provides support and motivation to improve individual wellbeing/hauora through physical activity and lifestyle support.
Any adults who are medically stable and are motivated to increase their physical activity and make some lifestyle changes
We are particularly keen to support anyone with either newly diagnosed or long-term health conditions who would benefit from increasing their physical activity levels eg Clients with diabetes, cardiovascular or respiratory conditions, osteoarthritis and/or depression and anxiety.
Complete a Green Prescription self-referral form (see below) - or speak with your doctor, practice nurse or health care professional, and ask for a Green Prescription.

Initial Consultation
Your advisor will contact you to let you know that we have received a referral for support. They will work with you to arrange an in-person meeting or a suitable time to have an initial phone conversation and start the support process. They will discuss your individual circumstances and needs and work with you to come up with an achievable action plan that you feel confident in. Support can also be provided online if preferred.
Ongoing Support and Review
Your advisor will stay in regular contact by phone, email or text to help keep you motivated and check in on progress with your plan. They will discuss the benefits you may have noticed, your activity levels and possible next steps. Prescription support periods aim to be responsive to need and renewals of prescriptions can be discussed with your advisor. Additional cultural support for Maori is available. If this is of interest to you please discuss with your advisor in the first instance.
Green Prescription support in the Community
The Green Prescription team, work with community activity providers in advocating for our clients to address some of the barriers we know exist when trying new types of physical activity and establishing new habits. We are grateful for providers who support our programme in many different ways. Please look out for the green prescription logo in our Activity Directory which identifies providers where support is available or talk with your Advisor about other opportunities.
